The player controls a robot with the ability to fly, but is still affected by gravity. He has seven weapons to choose from, and can perform melee attacks using a baseball bat or sword. The game has 160 levels, and features a level editor to create more. Edited levels can be encoded in a sound that can be recorded with a computer and shared between players. This technology was inspired by the datacasettes used by some older computers, including the Sharp X1 on which Matsumoto learned to program.

Dans Bangai-O Spirits, le joueur contrôle Bangai-O, un robot de combat possédant la capacité de voler et devant faire face à des hordes d'ennemis, le but du jeu étant de détruire dans chaque niveau un certain nombre de cibles. Pour cela, le joueur peut se servir et combiner les effets de différentes armes. Le jeu contient plus de 160 niveaux et intègre un éditeur permettant au joueur de créer ses propres aires de combat. Ces niveaux peuvent ensuite être échangés entre consoles.

Es ist der Nachfolger von dem Nintendo-64- und Dreamcast-Titel Bakuretsu Muteki Bangai-O. Bangai-O Spirits bietet über 100 Level. Ziel jeden Levels ist es eine Reihe von markierten Gegnern auszuschalten, wofür dem Spieler eine Reihe von Schuss-, Nahkampf- und Spezialattacken zur Verfügung stehen. In Bangai-O Spirits gibt es, anders als in Spielen sonst üblich, nichts Freischaltbares, alle Level und Waffen stehen von Anfang an dem Spieler zur Auswahl. Von einem Tutorial abgesehen gibt es keinerlei konkrete Handlung in dem Spiel.
