It is the sequel to the 1988 arcade game Splatterhouse, and the third installment in the Splatterhouse series, following Splatterhouse: Wanpaku Graffiti.
On August 4, 2008, the game was released on the Wii Virtual Console in North America. It is the first and the only ESRB M-rated game to be released for the Virtual Console. The game was included as an unlockable extra in the 2010 remake. It was included on the Sega Genesis Mini 2 in October 2022.

同社のアーケードゲーム『スプラッターハウス』（1988年）の家庭用オリジナル続編。主人公のリックを操作し、前作の舞台となった「スプラッターハウス」に再び潜入して亡き恋人ジェニファーを復活させる事を目的としている。基本的なゲームシステムは前作を踏襲しているが、敵キャラクターやギミックはすべて一新されている。
開発はナウプロダクションが行い、プログラムはゲームボーイ用ソフト『ミッキーズ・チェイス』（1991年）を手掛けたおおほりはるお、音楽はゲームボーイ用ソフト『ディグダグ』（1992年）を手掛けた金田栄子が担当している。
2008年にWii用ソフトとしてバーチャルコンソールにて配信された。
