The game utilizes a side-scrolling feature that allows the player to take control of either a male (Ricky) or female (Mary) soldier whilst hunting aliens and saving hostages before they run out of time. Alien Syndrome was released to commercial success and positive reviews, with praise towards its gameplay, character designs, sound design and horror themes. It was ported to various home computers and consoles, including Sega's own Master System, becoming one of the best-selling titles for that console. It was included as a bonus title in the 2009 compilation Sonic's Ultimate Genesis Collection. A sequel of the same name was released in 2007 to a mostly negative response from critics.

È uno sparatutto a scorrimento multidirezionale, tipico esponente del sottogenere run 'n' gun; si combatte a piedi dentro strutture infestate da alieni dall'aspetto particolarmente orripilante. Uscirono conversioni per numerose piattaforme, a cominciare da quella per Sega Master System prodotta dalla stessa SEGA. Nel 1988 vennero pubblicate versioni per gli home computer occidentali Amiga, Amstrad CPC, Atari ST, Commodore 64 e ZX Spectrum dalla ACE, etichetta usata dalla britannica Softek per alcune conversioni di arcade. Seguirono versioni di vari editori per MSX, MS-DOS, NES, Sharp X68000 e Sega Game Gear. Un rifacimento con grafica 3D per PlayStation 2 uscì nel 2004 nella serie Sega Ages. Il videogioco è incluso nelle raccolte Sega Classics Collection per PlayStation 2 (sebbene sia rimosso nella versione europea) e Sega Mega Drive Ultimate Collection per Xbox 360 e PlayStation 3. Nel 2007 è stato realizzato il seguito Alien Syndrome, ambientato un secolo dopo gli eventi del primo titolo.
